Medical Records
Requesting Medical Records
Medical records may be requested by filling out a Release of Medical Information Form signed by the parent/guardian. If the patient requesting the records is 18 years of age or older, they will have to sign the release form. There is a two week turn around time from the date our office receives the valid request. We do not print any records, records will be faxed or emailed. Completed release of medical information forms can be emailed to frontoffice@ppadocs.com
- Designated Records include any records from our office, including Progress Notes, Immunizations Records, Growth Charts and Labs.
- Complete Medical Records will include the patient's entire chart. These records have a flat fee of $17.50.
If you are needing your child's immunization records, these can be found on the portal for you to print.
